Request for Radioactive Waste Removal Form

 Waste pickups are scheduled every Tuesday morning except when the University is closed. Email waste removal requests need to be received by 7:00 AM Tuesday to assure a waste pickup that day. As in the past any phone requests need to be received by Monday at 4:00 PM.

Complete one of these requests for Each Location radioactive waste is to be picked up from.
