Inventory records must be kept for all unsealed radioactive material at Cornell University. The following documents are provided to assist inventory record keepers to understand the requirements and calculate activities correctly. Waste Disposition and Decay Excel Spreadsheet
This Excel file may be used to save time in calculating decay and activity. The spreadsheet is designed for individuals using the EH&S recommended method of tracking stock material in volume and waste disposition in percentage.

Unsealed Radioactive Materials Inventory Form (pdf)
Since the inventory forms for each stock vial are delivered with the vials, this form is needed only under special circumstances. This pdf format of a blank inventory form is provided should you need one, such as for an authorized transfer of material to another permit.
