APPENDIX B :
Respiratory Protection Certification Protocol

1. Respiratory Request Form

Complete section A of the Respiratory Request Form (Appendix C) with your supervisor's signature and submit it to EH&S.

2. EH&S Evaluation

EH&S personnel will determine if the use of a respirator is necessary for the appropriate work operation. This may be evaluated by one or a combination of the following methods:

  • Consultation with the Supervisor.

  • Interviewing the employee.

  • Observing the work operation.

  • Collecting air samples during the work process to assess airborne exposure to any toxic material. Respirators will be required for all operations where the atmospheric concentration is in excess of the limits specified by the Occupational Safety and Health Administration, American Conference of Governmental Industrial Hygienists, or as deemed necessary by EH&S.

  • Evaluating existing or alternative engineering controls.

3. Medical Approval

Obtain and complete from EH&S the Medical Evaluation Questionnaire for medical approval to wear respiratory protection. Send to Occupational Health Services at Gannett Health Center for medical review and approval. This part of the program has been developed by a physician from the Department of University Health Services. Further medical evaluation and medical surveillance may be necessary to determine if a person can wear respiratory protection equipment.

4. Respiratory Protection Training

EH&S will provide training on the use of respiratory protection at the time of initial assignment and fitting of the respirator. Anyone requiring the use of a respirator must receive training. The purpose of the training is to explain the limitations of the various respirators and to gain a better understanding on using the appropriate respirators and cartridges for the job.

5. Respirator Fit Testing

EH&S will provide quantitative or qualitative fit testing for each respirator issued. A respirator that provides the best comfort and protection will be issued to you along with the appropriate cartridges.

6. Respirator User's Responsibilities

  • Update your respirator qualification status annually.

  • Reschedule for a future date if you cannot attend your scheduled appointments.

  • Inform your supervisor of any personal health problems that are or could be aggravated with the use of respiratory equipment.
