In the laboratory or animal handling area, there are a number of practices and safety equipment that can be incorporated to reduce or eliminate biological hazards. Also, various practices must be in place to satisfy regulatory compliance. To help personnel accomplish these objectives I have put together a biological safety self-audit form. On this form I have provided a list of standard and special practices and safety equipment, followed by a description and/or ways to comply with or satisfy that condition. I have also included web links from pertinent references that can provide supporting information.
I think that the most important use for this form is the potential to educate and familiarize individuals with appropriate procedures in a biological laboratory. In order to accomplish that this self-audit should be conducted at least every 6-12 months (or more often if you prefer), and with every new employee or student.
